The Brain of Cthulhu is a pre-Hardmode , Crimson -themed boss . Fighting the boss and its attendant Creepers is the only way to obtain Tissue Samples , and is one of the ways to obtain usable quantities of Crimtane Ore . Defeating the Brain of Cthulhu for the first time also allows the Tavernkeep NPC to spawn as the Unconscious Man, and allows meteorites to land. The Brain of Cthulhu is the Crimson counterpart to the Corruption 's Eater of Worlds . Unlike its Corruption counterpart, however, it does not have a mechanical equivalent in Hardmode . While the Brain of Cthulhu is alive, the music Boss 3 will play. When the first three Crimson Hearts are shattered in a world born of blood, the Brain awakens from its slumber, initially appearing as an invincible phantom that dances just beyond the reach of mortal steel. It is only when its legion of Creepers is utterly extinguished that the creature's true horror is revealed: a pulsating heart exposed within a cracking skull, vulnerable yet ferocious. This entity has no mechanical successor in the Hardmode era, standing as a singular testament to the Crimson's unique brand of madness, guarding precious Tissue Samples and the secrets of the mind itself.
In Their Own Story
You stand at the edge of the chasm, the red glow casting long, dancing shadows against the cavern walls. The air is thick with a scent that is not quite rot, but something far more ancient and organic. Suddenly, the silence shatters as the Brain of Cthulhu materializes above you, a translucent sphere of writhing nerves. It does not attack; it simply hovers, ignoring your strikes as if they were mere whispers against a hurricane.
Twenty spectral Creepers erupt from the darkness, their eyes glowing with an eerie luminescence. They orbit the brain in chaotic spirals before hurling themselves at you, screaming without mouths. You swing your sword, severing limb from phantom body, but for every one you destroy, the Brain teleports closer, laughing in a silence that echoes inside your skull. Finally, the last Creeper dissolves into mist. The Brain shudders, its outer shell cracking open to reveal the raw, beating heart within. It is no longer invincible. It lunges, teleporting wildly across the arena, trying to crush you with its exposed mind, but you stand firm, your blade ready to sever the connection between this nightmare and the world of men.
Reader's Guide
Summon the Brain by shattering three Crimson Hearts or using a Bloody Spine in The Crimson. Prepare an arena with multiple long platforms spanning at least two screens, ensuring clearance for the Creepers' sweeping orbits.
In Phase One, the Brain is invincible and teleports while Creepers attack. Focus entirely on eliminating all twenty Creepers; they do not respawn. Use ranged weapons or high-damage melee attacks to hit groups as they converge. Note that Creepers drop Hearts at a 50% rate.
Once the Creepers fall, Phase Two begins: the Brain becomes visible and vulnerable. It will aggressively close distance and teleport rapidly. In Expert Mode, three mirror images appear, mimicking its movements and becoming opaque as health drops. Dodge by moving perpendicular to its charge or using wings/vertical mobility. Equip accessories that mitigate confusion or provide defense. The Brain of Confusion is a crucial drop from the Treasure Bag in Expert mode. Remember, leaving the Crimson biome will cause the boss to despawn.

- Defeating the Brain is the only way to unlock the Tavernkeep NPC and trigger meteorite landings.
